Carers Self Assessment
What is the Carers Self Assessment?
The Carers Self Assessment is your chance to tell us about your support needs as a carer and the things that you think will help. It is not a replacement for a full carers assessment, however it can be used to provide short – term help in a timely manner.
The Carers Self Assessment is a short questionnaire about the care you provide, and the support you currently receive. From this information, we are able to assess how we, the Carers Centre, are able to support you further in your caring role. The support we can offer varies, from benefits advice, to accessing funding for you to partake in a hobby or interest, to a short term sitting service. The sitting service and funding available is only available on a short term basis, and is based on individual needs and eligibility. Please contact the Carers Centre should you wish to discuss this further.
Case Study
Mrs A cares for her mother full time. She has seen a 6 week computer course that she would like to do, but does not have the money to pay for the course, nor does she have someone else to care for her mother whilst she is attending the course. After completing the carers self assessment, Mrs A was given access to some funding to pay for her course, and was also offered a sitting service to look after mum whilst she was studying.
How to complete the assessment
You can complete the self assessment on line. Just follow this link- carers self assessment online. Alternatively, you can download the form and print it off to complete Carers self assessment form 2011-2012 (NB: Please read the front page before you complete the form carers self assessment front page 2011-2012 ) If you need support to complete the form, a member of staff from the Carers Centre would be able to assist you. We are also able to accommodate for people who have difficulty understanding English via a translation/ interpretation service. Please contact the Carers Centre for more details.
