How to Refer

Referring a carer or young carer to the centre couldn’t be easier. Simply download the appropriate form below, complete and email back to carerscentre@portsmouthcc.gcsx.gov.uk (NB: This email address is a secure account).

Alternatively you can print the form out, completed with the carer and return via post to the centre (our address is at the bottom of the homepage).

If you have any questions about a referral please contact the centre on 023 9285 1864 or 023 9275 6780, or email carerscentre@portsmouthcc.gov.uk (NB: Please do not send confidential information to this email address but use the address listed above). Please note, the below adult mental health (AMH) referral form is for professionals only. If you are an AMH carer and would like support, please contact the team directly either via email, telephone, post or drop into the centre.
